Working on this project as a Master's of Health Administration Student through the University of Oklahoma College Health Science Center - Hudson College of Public Health has allowed me to collaborate interdisciplinary with those across our campus. Throughout this project, I have been able to innovate novel methodologies to design a care delivery model that seeks to incorporate Social Determinants of Health and Health Literacy at the patient bedside as a part of the patients discharge planning utilizing my skills in healthcare administration to organize, coordinate, and implement the desired strategy.

To address gaps in clinical education between the University of Oklahoma Colleges of Medicine and Pharmacy, a novel curriculum was initiated with focus on the exploration of health inequity, community health, health literacy, and social determinants of health (SDoH).
