I have been a co-investigator on several federally funded trials focused on improving IP geriatric practice and integrating medical and community-based providers. Interventions in these trials ranged from chronic illness management to elder abuse to end-of-life care. We established our ability to integrate geriatrics and primary care and to track MIPS outcomes. Our ability to intervene and positively impact outcomes at the community level is similarly well-established. On our current GWEP, we have gained the expertise needed to expand the educational and clinical models we have established that have positively impacted patient outcomes through integration of geriatrics and primary care.

We will describe a 40-hour Geriatric Resource Course (GRC) for interprofessional learners from healthcare and community provider settings implemented as part of a HRSA-funded Geriatric Workforce Enhancement Program (GWEP). This program is designed to promote interprofessional collaborative practice in geriatrics. It originally combined online didactics, in-person lectures and experiential trainings at sites spanning healthy aging through end of life.
