Susan M Fosnight is a Professor in the Department of Pharmacy Practice at Northeast
Ohio Medical University with a joint appointment as a Clinical Lead Pharmacist in
Geriatrics at the Department of Pharmacy at Summa Health System.
Sue provides clinical pharmacy services to both inpatients and outpatients at Summa
Health System. She serves as a consultant to the geriatric consultant service and provides
care to the House Calls service. Sue provides education to pharmacy, medicine, and nursing students. She serves as the
pharmacist resource for the Geriatric Workforce Enhancement Grant.

The traditional model of medical care only accounts for 10% of health outcomes, whereas social determinants of health account for over 60%. Since older adults are commonly diagnosed with multiple chronic illnesses, effective care for these patients requires meaningful collaboration between medical and community-based care providers. We will describe an integrated interprofessional collaborative model of care between a health system and the local Area Agency on Aging (AAoA) called the Care Management Interdisciplinary Team (CMIT).
