Danielle Clay, a Cleveland State University graduate, is a Licensed Social Worker with over 20 years of experience working within the Long Term Care sector. Danielle began her career with Western Reserve Area Agency on Aging (WRAAA) over 10 years ago, starting as a Care Manager for PASSPORT, then transitioning to a Supervisor for the MyCare Ohio program before being promoted to Clinical Manager of the Ohio Home Care Waiver/SRS programs. Currently, Danielle manages the Training & Development department at WRAAA.

The traditional model of medical care only accounts for 10% of health outcomes, whereas social determinants of health account for over 60%. Since older adults are commonly diagnosed with multiple chronic illnesses, effective care for these patients requires meaningful collaboration between medical and community-based care providers.
