Brandi joined Direction Home in August of 2003 and has held multiple positions within the agency over her 18 year tenure. She helped develop two successful transitions programs which assisted individuals living in nursing facilities to move back into their community setting of choice, while providing home and community based support and services to allow them to remain safely, successfully and sustainably in their homes. Her most recent transition program earned accolades from the Ohio Department of Medicaid as being the most successful transition program in the state.

The traditional model of medical care only accounts for 10% of health outcomes, whereas social determinants of health account for over 60%. Since older adults are commonly diagnosed with multiple chronic illnesses, effective care for these patients requires meaningful collaboration between medical and community-based care providers.
